Together with your team, you will look at the goals you would like to achieve and develop a Person-Centered plan. This plan will act as a map to guide your Team in using your existing strengths and supports to reach your goals. This plan will also assist your Team in identifying the supports and resources that are needed to identifying the supports and resources that are needed to make your life the way you want it to be. Your case Manager will guide your Team in reviewing your plan on a regular basis and to monitor and change the plan, as needed.

It may take a while to see results but, after your new services and supports are started, you should begin to see and feel a difference. Sometimes changes are noticeable very soon but others may take more time. Any time you wonder if your Person-Centered Plan is working, we suggest you talk with your team and get their opinions. Remember you can call your Case Manager at any time and request a team meeting to review your goals, supports, resources, and progress.

Your Team will assist you in writing a Crisis Plan that will give you a clear plan of action so that you and /or your family will know how to prevent emergencies, or get help in an emergency at any time, day or night.
A Crisis Plan is important because it helps everyone involved to cope in a crisis situation. The Crisis Plan also help everyone to respond in a timely way to keep a problem from getting worse, and it helps to return your life to normal, as quickly as possible.

If, at any time, you have concerns about services you are receiving, you have the right to have your concerns or complaints heard. Both The Loving home and the Local Management Entity (LME) have established grievance procedures to address your concerns and complaints in a timely manner. The desired outcome of this process is to improve your satisfaction with the services you are receiving.
